If you’ve ever opened the Katie Beckett renewal form in Georgia and immediately felt overwhelmed, you’re not alone. The form is 16 pages long and can look intimidating — but most of it doesn’t even apply to you. In this post, I’m walking you through it page by page: what to fill out, what to skip, and what to attach when you mail it in.

This walkthrough is based on my own renewal experience with two kids on Katie Beckett. I’m not guessing at this — I actually did it, and I’m sharing exactly what my completed form looked like so you can see what to expect. Before You Start: The New Georgia Katie Beckett Portal

Georgia launched a brand new online portal for Katie Beckett members on April 15, 2026. You can find it at katiebeckettportal.dch.georgia.gov. If your case has been entered into the new system, you’ll receive an email invite granting you access.

I finished my renewal right before the portal went live, so this walkthrough covers the paper Katie Beckett renewal form in Georgia. If you haven’t received a portal invite yet or you’re doing a paper renewal right now, keep reading — this is for you.

About Form 508

The Katie Beckett renewal form in Georgia is Form 508 — a general renewal form used across multiple Georgia programs including TANF and SNAP. Because it covers other programs, the majority of it has nothing to do with Katie Beckett. The Katie Beckett office told me directly which sections to fill out and which to ignore, and that’s exactly what I’m sharing here. But, to be safe, always get instructions from the Katie Beckett office in case things have changed.

Since I have two children that receive Medicaid via the Katie Beckett Deeming Waiver, the office told me that I could fill out one application, but include both children’s info as needed. Also, it’s important to note that the information that I am providing below is what is true for my family and some answers may be different for your family.

Katie Beckett Renewal Form in Georgia: Page-by-Page Walkthrough

Page 1: Basic Information

Skip the “For Office Use Only” box at the top. Then fill in:

  • Client name — I listed both of my children
  • Date of birth and Social Security number for each child
  • Are you homeless? — No
  • Street address and mailing address
  • Main phone number and alternate number
  • Email and texting permissions — optional, but I allow both
  • Preferred language — ours is English
  • Interpreter needed — No (if not applicable)
  • ADA disability — No (if not applicable)

Page 2: Sign and Date

Sign and date this page as the legal guardian.

Page 3: Authorized Representative and Tax Information

Skip the Authorized Representative section at the top entirely — unless someone else is filling out the form for you.

Scroll down to the Medicaid Only section at the bottom and fill out the tax questions:

  • Do you expect to file a federal tax return next year? — Yes
  • Will you file jointly with a spouse? — Yes, list spouse name
  • Will you claim dependents? — Yes, list children’s names
  • Will anyone be claimed as a dependent on someone else’s return? — No (unless this applies to your situation)

Page 4: Citizenship and Household Chart

Fill in the citizenship information at the top of the page, then complete the household chart. List your Katie Beckett children first with Relationship listed as SELF, then the rest of your household. For each person include:

  • First name, middle initial, last name
  • Race, sex, date of birth
  • Relationship to the Medicaid recipients
  • Social Security number
  • US Citizen column — Y for all (if applicable)
  • Mother/father in home — Y or N as applicable
  • “Do you want Medicaid?” — Y for your Katie Beckett children, N for everyone else

Page 5: Naturalized Citizen Chart and Foster Care

If everyone in your household is a US citizen, skip the naturalized citizen chart and leave it blank.

Answer the foster care question: “Was anyone in your household in foster care at age 18?” — No for us, but fill it in if it applies. Leave the rest of the page blank — it’s for SNAP and TANF only.

Page 6: Unpaid Medical Bills

Answer Question 5: “Does anyone have any unpaid medical bills for the last 3 months?” If yes, attach those bills to your application. Leave the rest of this page blank — it’s for SNAP and TANF only.

Page 7: Skip

This page is for SNAP and TANF only. Skip it entirely.

Page 8: Health Insurance and Resources

Fill out the Other Health Coverage section:

  • Check: Georgia Department of Human Services Medicaid
  • Check: Employer Insurance — write the insurance name and policy number if you also have private insurance
  • “Do you have health insurance other than Medicaid?” — Yes, if you have another insurance.
  • Attach a copy of your insurance card (front and back) at the end of your application

For the Resources section, check No. In the Cash row, I wrote a note: “[Children’s names] have no resources in their name.” This helped clarify things for the reviewer. Skip the Aged, Blind or Disabled Medicaid section at the bottom. These other ways to qualify for Medicaid don’t apply for those with the Katie Beckett Deeming Waiver.

Page 9: Employment

“Does anyone work?” — Yes. Fill in the employed person’s name, employer, pay per hour, hours per week, how often they’re paid, and recent pay dates. Then answer:

  • Is anyone on strike? — No
  • Pre-tax expenses: fill in health, vision, and dental insurance amounts and frequency (this is on their pay stub)
  • Has anyone stopped working? — No
  • Has anyone started working? — No

Answer based on your actual situation — these are just our answers.

Page 10: Self-Employment and Other Income

I was not sure if this section was necessary for me, but since the Katie Beckett office told me to fill out the Employment section, I decided to include this. If you are self-employed, you may want to clarify with them. Fill in your name, note whether your business is incorporated, and list your self-employment expenses. I attached my Schedule C from our most recently filed taxes as proof.

If you are not self-employed, skip this section.

For unearned income (Social Security, SSI, child support, unemployment, etc.) — answer No if it doesn’t apply. If it does, attach proof.

For dependent care: answer Yes only if you pay someone to watch your child so you can work. The transportation question here refers to that same childcare arrangement — not medical appointments.

Page 11: Shelter Costs and Child Support

Skip shelter costs — the Katie Beckett office instructed me to leave that section blank. For child support, answer Yes or No. If Yes, fill out the chart provided.

Page 12: Skip

Not required for Katie Beckett. Skip it.

Pages 13–14: Read Your Rights

Nothing to fill out here, but take a few minutes to read through your rights and the program warnings. It’s worth knowing what’s in there.

Page 15: Voter Registration

This section relates to your child, who isn’t voter age. Leave it blank.

Page 16: Signatures

There are three signature boxes on this page. Use the first one only — it says “For Medicaid only — sign here when the Applicant/Member/Legal Guardian is completing the form.” Sign and date there.

Skip the second box (for someone other than the applicant) and the third box (SNAP and TANF only). Neither applies to Katie Beckett.

What to Attach

Once your Katie Beckett renewal form in Georgia is complete, gather your supporting documents before you seal the envelope. For our family that meant:

  • Private insurance card — front and back
  • One month of the most recent pay stubs
  • Schedule C self-employment tax form (if self-employed)

What you attach will depend on what you filled out. Include whatever supports your answers on the form.

Tips After You Mail the Katie Beckett Renewal Form in Georgia

The Katie Beckett office is short-staffed and processing times are slow. Here’s what I’d recommend once your application is in the mail.

Keep your post office receipt. Get tracking when you mail it so you have proof it arrived. It took about a month for them to process my application after it was received — but they did eventually get to it.

Call at 8 a.m. The best time to reach the Katie Beckett office is right when they open. You still might not get a live person, but you’re more likely to get a callback.

Your coverage should stay active. Coverage should remain in place until a determination is made on your Katie Beckett renewal form in Georgia, and they should backdate it if it does lapse. If your Medicaid number becomes inactive during the process, call every morning at 8 a.m.

Escalate if needed. If you can’t reach anyone after about a week, go to medicaid.georgia.gov, scroll to the bottom, and use the “How can we help you?” contact form. This reaches DCH constituent services. In my experience I usually hear back within a day — though with my most recent renewal it took two attempts before someone called me back.
